Risk Assessments

Risk Assessments in Formbird FLEET are completed as part of the workshop and maintenance workflow. They provide a structured way to identify and control hazards before maintenance or repair work is carried out on an asset.

There are two ways to complete a Risk Assessment, depending on how it is used:

  • Risk Assessment Work Order — a standalone Work Order type created within a Request, used when a risk assessment is the primary task.
  • Risk Assessment Questionnaire — completed as part of a Repair Work Order, when a Repair Type has been configured to include a risk assessment questionnaire.

Completing a Risk Assessment Work Order

A Risk Assessment Work Order is created and managed within a Request, the same way as any other Work Order.

  1. On a Request, create a Risk Assessment Work Order.
  2. Open and manage the Work Order workflow as usual.
  3. Answer the configured risk questions. These are organisation-specific and set up in the Risk Assessment configuration.
  4. If additional risks have been identified that aren't covered by the questions, add them using the Other Risk Identified option.
  5. Select Create Risk Items. This generates risk items only for questions with a Yes response and for any other risks identified.
  6. Select a risk item from the grid to open it.
  7. Assess the risk and select the applicable controls.
  8. Save the risk item and move to the next item in the grid. Repeat until all items are assessed.
  9. Once all items are complete, select whether the Risk Assessment Passes or Fails from the dropdown — this records whether all risks fall within the organisation's acceptable threshold.
  10. Sign the document.

Completing a Risk Assessment Questionnaire

Risk Assessment Questionnaires are linked to specific Repair Types. When a Repair Work Order uses one of those types, the questionnaire becomes available to complete.

  1. Create a Repair Work Order using a Repair Type that has Risk Assessment Questionnaires configured.
  2. Select Risk Assessment Questionnaire from the questionnaires grid.
  3. Select Create Questionnaires.
  4. From the created questionnaires grid, open the desired questionnaire by selecting it.
  5. Complete the questionnaire following the same steps as a standalone Risk Assessment Work Order (steps 3–10 above).
