Risk Assessment Configuration

Risk Assessments require several configuration steps before they can be used. Most steps are completed by an Administrator using the Configuration Menu; the Risk Matrix is set up with assistance from Formbird Support.

Configuration Steps

1. Configure the Risk Matrix

The Risk Matrix defines how risk is rated across the organisation — the likelihood and consequence scales and the thresholds that determine a pass or fail outcome.

Contact Formbird Support to configure the Risk Matrix for your environment.


2. Create Risk Assessment Questions and Control Measures

Risk Assessment Check Questions and Control Measures are managed from the Daily Driver / Risk Assessment page in the Configuration Menu.

  • Questions represent the potential hazards that assessors will respond to.
  • Control Measures are the available responses to each risk — the controls that can be selected to demonstrate a risk is managed.
  • The same categories used for Daily Driver questions and Risk Assessment questions and Control Measures are shared across both.

3. Add Questions to Asset Group Mapping Documents

Questions are assigned to asset types via the Asset Group Mapping documents. This controls which risk questions appear for a given type of asset.

Navigate to the appropriate Asset Group Mapping document and add the relevant questions for that asset type.


4. Enable Risk Assessment Work Orders on an Asset Type

For Risk Assessment Work Orders to be available on Requests for a given asset type, the Work Orders field on the Asset Group Mapping document must include the Risk Assessment Work Order type.


5. Add Asset-Specific Checks (Optional)

If individual assets require additional checks beyond those configured for their asset type, these can be added using the Add a new child document field on the single asset mapping document.


6. Enable Risk Assessment Questionnaires on Repair Types (Optional)

To make Risk Assessment Questionnaires available on Repair Work Orders, navigate to the applicable Repair Type(s) and add Risk Assessment Questionnaire to the Related Questions field.
